About the position Responsibilities • Partner with a registered nurse case manager to assess and plan for members' health needs. • Research available options and resources for members with financial, transportation, safety, and housing needs. • Empower members to make positive lifestyle and medical choices. • Assist members with community resources for medical, financial, and behavioral health. • Conduct assessments and provide support to a diverse population throughout their lifecycle. • Engage in telephonic and electronic communication with members. Requirements • Must have a Master's Degree in Social Work (MSW) from an accredited program. • Experience in healthcare social work or a related field is preferred. • Strong communication skills, both verbal and written, to effectively interact with members and clinical staff. • Ability to work independently and manage time effectively in a telework environment. Nice-to-haves • Experience with case management and advocacy in a healthcare setting. • Familiarity with community resources and support services available to members.
